I haven't seen one book with both. Tere Hagen has a book out on each, however, and while you won't find obscure patterns in them, they do cover most of the popular ones.
Maryanne Dolan, professional expert (meaning 'doesn't specialize in silver'

, has a book out called _American Sterling Silver Flatware 1830 - 1990_. It's not all that one could hope for but it is cheap, around $20.
Richard Osterberg recently brought out the second edition of his _Sterling Silver Flatware for Dining Elegance_ (hardcover, around $40). Poor guide to pattern identification but useful for identifying unusual pieces in the popular patterns since it has pictures and measurements.
Kathleen Ellis has a _Sterling Flatware Pattern Index_, which is rather pricey. I haven't seen it so can't comment.
Finally, a company called Eden Sterling has gotten hold of old flatware catalogues and made copies of these, which they hawk with a notable lack of success on yabe. (Could be because people are unwilling to pay book prices for a bunch of photocopied sheets.) If there's a particular pattern you're interested in and you want to know what all the shapes are, this is one route to go.
